Eclipsys Knowledge-Based Charting Helps Health First Improve Patient-Care Outcomes

BOCA RATON, Fla., Sept. 26 /PRNewswire-FirstCall/ -- Eclipsys Corporation(R) , The Outcomes Company(R), today announced that Health First, an integrated healthcare delivery system in Rockledge, FL, has achieved notably improved patient-care outcomes using Eclipsys' Knowledge- Based Charting(TM) solution. Health First's results include reduced duplicate documentation, improved interdisciplinary communication and increased patient safety through evidence-based care.

Comprised of three not-for-profit hospitals -- Cape Canaveral, Holmes Regional Medical Center and Palm Bay Community Hospital -- Health First, named one of the "Most Wired Hospitals" the last three years, implemented Eclipsys Knowledge-Based Charting to supplement an organization-wide effort to realize its vision of setting the standard for safe, effective, patient-centric care.

To achieve its vision, Health First collaborated with Eclipsys consultants to establish Partnership Councils -- a series of inter-departmental unit-level groups -- that were responsible for guiding the clinical transformation process and customizing a sub-set of guidelines appropriate for each specialty. With their assistance, the Councils worked to drive a cultural change with the staff oriented around evidence-based individualized care plans, standardized practices, and multi-specialty collaboration to ensure adoption across the entire organization.
Within one year, 2,800 clinicians at all three hospitals and ancillary departments-including pharmacy, transcription, ED, labs and radiology-were trained on and had adopted the Eclipsys solution to document patient care across all departments. Since completing the implementation and going live, Health First/Cape Canaveral Hospital has achieved numerous clinical and patient safety outcome improvements, including:
-- Reduction in patient fall rates: * 58% five months post implementation * 92% one year post implementation -- Reduction in pressure ulcer rates: * 55% reduction in pressure ulcer rate post-implementation; maintained reduction two years after go-live * Maintained zero percent pressure ulcer rate for wound sweep post implementation -- Improved efficiency through significant reduction of duplicate documentation -- Recent Joint Commission survey found no RFIs (requirements for improvement) for any patient safety goals
